Potential for Fraud: A majority of these cases involved names or birthdays that did not match the deceased person, suggesting potential synthetic identity fraud, where real SSNs are combined with fake information.

Vulnerability: GAO investigators were able to enroll fake applicants into the system with minimal scrutiny and without providing proper documentation, highlighting weaknesses in the verification process.

Ongoing Issue: The findings for 2023 and 2024 indicate a pattern of vulnerabilities, with unauthorized changes made by insurance agents and brokers also being a significant issue.
