JD Vance just told the Attorney General that certain doctors belong in prison.
The Vice President fired off a letter to AG Todd Blanche after a bombshell Department of Health and Human Services investigation uncovered what officials describe as potentially fraudulent billing tied to pediatric gender treatments. Vance formally referred the findings to the DOJ for possible criminal prosecution, and he did not mince words about what should happen next.
Here is the jaw-dropping scope of what HHS found. Over 225 hospitals and health systems across America launched pediatric gender programs. Insurance claims data spanning 2015 to 2025 revealed roughly $50 million in charges for puberty-blocking medications coded under a vague diagnosis of unspecified endocrine disorder. Another $11 million in claims involved teens aged 13 to 17 billed under a precocious puberty code, a diagnosis HHS says would almost never apply to patients that old.
And it gets wilder. Since 2019, all-payer claims data showed close to $120 million in charges for related procedures on minors. That includes more than 5,500 surgeries and around 8,500 courses of hormones or puberty blockers.
Vance zeroed in on the suspicious coding in his letter to Blanche. He cited one study showing only about 4.7 percent of patients tagged with the unspecified endocrine disorder code actually had that condition. He also pointed to a 30 percent spike in those diagnoses, despite zero evidence that pediatric endocrine disorders are actually becoming more common. Translation: somebody was gaming the billing system.
The financial incentives are staggering when you look at the numbers. HHS noted that average yearly healthcare costs for a minor sit around $3,000. But treatment pathways involving these interventions could balloon to roughly $75,000 without surgery and a whopping $170,000 when surgical procedures are part of the equation. That is an enormous revenue jump per patient.
HHS Secretary Robert F. Kennedy Jr. also got in on the action, sending a separate batch of suspicious claims to the department's Inspector General. Those flagged claims include puberty blockers paired with vague endocrine diagnoses that lack any accompanying gender dysphoria or precocious puberty codes for patients between 9 and 17. Kennedy's referral also covers prescriptions written in states that have already banned these treatments for minors.
Vance's position could not be clearer. He wrote that providers who intentionally miscoded treatments to secure insurance coverage for procedures that would otherwise be denied need to face real consequences. Not a slap on the wrist. Not losing a license. Prison.
The referral comes on the heels of the Trump administration cutting off federal Medicaid and CHIP funding for these procedures on minors. So the financial pipeline is already getting squeezed.
As of now, the Justice Department has not publicly confirmed whether criminal investigations or prosecutions will follow. But with the Vice President of the United States personally dropping this on the AG's desk with a recommendation of incarceration, it is hard to imagine this gathering dust in a filing cabinet.
The political pressure is enormous. The alleged dollar amounts are massive. And JD Vance clearly wants somebody in handcuffs.
